{"id":4138,"date":"2025-12-29T10:00:43","date_gmt":"2025-12-29T09:00:43","guid":{"rendered":"https:\/\/www.fondazionebonadonna.eu\/?p=4138"},"modified":"2025-12-29T10:01:24","modified_gmt":"2025-12-29T09:01:24","slug":"a-ctdna-guided-atezolizumab-therapy-in-bladder-cancer","status":"publish","type":"post","link":"https:\/\/www.fondazionebonadonna.eu\/en\/a-ctdna-guided-atezolizumab-therapy-in-bladder-cancer\/","title":{"rendered":"A ctDNA-guided atezolizumab therapy in bladder cancer"},"content":{"rendered":"<div class=\"wpb-content-wrapper\"><p>[vc_row][vc_column][vc_custom_heading text=&#8221;In patients with muscle-invasive bladder cancer, ctDNA-guided adjuvant therapy with atezolizumab improved disease-free survival and overall survival&#8221; font_container=&#8221;tag:h3|text_align:left&#8221; use_theme_fonts=&#8221;yes&#8221;][\/vc_column][\/vc_row][vc_row][vc_column width=&#8221;1\/2&#8243;][vc_column_text]Data from IMvigor011 phase 3 trial, <a href=\"https:\/\/www.nejm.org\/doi\/abs\/10.1056\/NEJMoa2511885\" target=\"_blank\" rel=\"noopener\">recently published on The New England Journal of Medicine<\/a>, showed that a ctDNA-guided adjuvant therapy with atezo\u00adliz\u00adu\u00admab led to significantly longer disease-free survival and overall survival than placebo in patients with muscle-invasive bladder cancer.<\/p>\n<p>For the IMvigor011 trial, a total of 761 patients with muscle-invasive bladder cancer and no radiographic evidence of disease after surgery were enrolled and tested for ctDNA during surveillance, to detect molecular residual disease that may identify patients at high risk for recurrence after cystectomy who can benefit from adjuvant immunotherapy. Patients who tested ctDNA-positive underwent randomization, 167 to the atezo\u00adliz\u00adu\u00admab group and 83 to the placebo<\/p>\n<p>group. The median disease-free survival was 9.9 months with atezo\u00adliz\u00adu\u00admab and 4.8 months with placebo, the median overall survival was 32.8 months and 21.1 months respectively. Among 357 patients with persistent ctDNA-negative status, disease-free survival was 95% at the end of the 1-year monitoring period and 88% at 2 years. Atezo\u00adlizumab treatment was not associated with any new safety signals. As authors conclude, \u00abResults from the IMvigor011 trial indicate that serial<\/p>\n<p>ctDNA-based monitoring of molecular residual disease can identify patients with bladder cancer who may benefit from adjuvant atezolizumab while sparing patients who persistently test ctDNA-negative from unnecessary treatment, toxicity, and costs.
